In-patient supports

We at Niagara Care Connects understand that sometimes clinical and medical intervention is required to ensure the health and safety of individuals. Our team is experienced in working within hospital and in-patient settings. 

Services offered:

• Direct Support and Engagement
• Positive Behavioural Supports
• Transition Planning and Supports
• Advocacy and Family Support
• Coordination with other Community Services
• Basic Personal Care
• Others based on individual needs. 

We ensure loved ones get the support they need so families can get back to being families. 

Hours of Service: 8 am - 8 pm, 7 days a week
Rates: Please contact for current hourly rates. Mileage and incidental costs are extra.
