Niagara Care Connects offers residential services out of our newly renovated home in Crystal Beach. Our residence is equipped to provide support in group, supported and transitional living capacities.
Additionally, we are able to offer support to individuals who already reside in their own residential location.
Services offered:
- Direct Support and Engagement
- Life Skills/Activities of Daily Living
- Community Access and Recreation
- Outcome-based, person-centered approaches and planning.
- Positive Behaviour Supports
- Basic Personal Care and Medication Support
- Transition Planning and Supports
- Others based on individual needs.
Hours of Service: 24/7, 7 days a week, including holidays.
Rates: To be determined based on individual service and support needs.
